Extramammary Paget’s disease is a rare pathologic entity. This is a case of a 66-year old male with 3 year history of a non-healing erythematous skin lesion at the right inguinopenoscrotal area which on skin biopsy turned out to be Paget’s disease. The patient underwent surgical excision with 1 cm margin and immediate reconstruction. Immunohistochemical staining was needed to confirm the diagnosis.
